AK RANA<\/strong><\/li>\n<\/ul>\n\n\n\n<p>Hypothyroidism and Hyperthyroidism are two types of thyroid disorders.<\/p>\n\n\n\n<p><em>Thyroid gland<\/em> releases thyroid hormones after getting stimulus from TSH (thyroid stimulating hormone) released by <em>pituitary gland<\/em>.<\/p>\n\n\n\n<p>TSH acts on the receptors present on thyroid gland and causes release of thyroid hormone.<\/p>\n\n\n\n<p>There can be two cases \u2013<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Increase<\/strong> in thyroid hormone called as <strong>Hyperthyroidism.<\/strong><\/li>\n<\/ul>\n\n\n\n<p>All the functions of thyroid hormone is increased.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Decrease<\/strong> in thyroid hormone called as <strong>Hypothyroidism.<\/strong><\/li>\n<\/ul>\n\n\n\n<p>All the functions of thyroid hormone is decreased.<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-pale-ocean-gradient-background has-background\" style=\"font-size:24px\"><strong><u>HYPOTHYROIDISM<\/u><\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"has-medium-font-size\">Thyroid hormone is involved in Basal Metabolic Rate (BMR) of body.<\/li>\n<\/ul>\n\n\n\n<p>In Hypothyroidism there is decrease in&nbsp; BMR.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Thyroid hormone increase glucose production by <em>glycogenolysis<\/em> and <em>gluconeogenesis<\/em>.<\/li>\n<\/ul>\n\n\n\n<p>In hypothyroidism \u2013 Hypoglycemia is an important finding ( especially in myxedema coma ).<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Thyroid hormone promotes lipolysis (breakdown of lipids).<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Thyroid hormone is involved in cholesterol clearance. In its deficiency <strong>hypercholesterolemia<\/strong> takes place.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading has-blush-light-purple-gradient-background has-background\" style=\"font-size:24px\"><strong>REGULATION OF THYROID HORMONE<\/strong><\/h2>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"885\" height=\"1024\" src=\"https:\/\/halleysclinic.com\/wp-content\/uploads\/2023\/05\/20230525_122108-1-885x1024.jpg\" alt=\"in hypothyroidism there is no negative feedback\" class=\"wp-image-87\" srcset=\"https:\/\/halleysclinic.com\/wp-content\/uploads\/2023\/05\/20230525_122108-1-885x1024.jpg 885w, https:\/\/halleysclinic.com\/wp-content\/uploads\/2023\/05\/20230525_122108-1-259x300.jpg 259w, https:\/\/halleysclinic.com\/wp-content\/uploads\/2023\/05\/20230525_122108-1-768x888.jpg 768w, https:\/\/halleysclinic.com\/wp-content\/uploads\/2023\/05\/20230525_122108-1-1328x1536.jpg 1328w, https:\/\/halleysclinic.com\/wp-content\/uploads\/2023\/05\/20230525_122108-1-1771x2048.jpg 1771w, https:\/\/halleysclinic.com\/wp-content\/uploads\/2023\/05\/20230525_122108-1-1024x1184.jpg 1024w, https:\/\/halleysclinic.com\/wp-content\/uploads\/2023\/05\/20230525_122108-1-600x694.jpg 600w\" sizes=\"auto, (max-width: 885px) 100vw, 885px\" \/><\/figure>\n\n\n\n<h2 class=\"wp-block-heading has-pale-ocean-gradient-background has-background\" style=\"font-size:23px\"><strong><u>HYPOTHYROIDISM IS OF TWO TYPES \u2013<\/u><\/strong><\/h2>\n\n\n\n<p><strong>CONGENITAL \u2013 <\/strong>Present since birth.<\/p>\n\n\n\n<p><strong>ACQUIRED \u2013 <\/strong>It is further of two types-<\/p>\n\n\n\n<p>Primary Hypothyroidism<\/p>\n\n\n\n<p>Secondary Hypothyroidism<\/p>\n\n\n\n<p><strong>Primary Hypothyroidism<\/strong> \u2013 It is defect or problem which lies in thyroid gland, which is resulting in decreased release of thyroid hormone in body.<\/p>\n\n\n\n<p><strong>Secondary Hypothyroidism<\/strong> \u2013 Defect lies at central level, in either <em>Hypothalamus<\/em> or <em>Pituitary gland<\/em>.<\/p>\n\n\n\n<p>Hypothalamus releases TRH (Thyrotropin releasing hormone) that acts on pituitary which further releases TSH that act on thyroid gland.<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-blush-light-purple-gradient-background has-background\" style=\"font-size:24px\"><strong><u>DIAGNOSIS<\/u><\/strong><\/h2>\n\n\n\n<p>When you go to clinic for TFT (Thyroid function test).<\/p>\n\n\n\n<p>They check two things usually-<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>T3 and T4 ( forms of thyroid hormone)<\/li>\n\n\n\n<li>TSH<\/li>\n<\/ul>\n\n\n\n<p>If it is Primary hypothyroidism then T3 and T4 will be decreased. It sends signal to pituitary to increase TSH.<\/p>\n\n\n\n<h1 class=\"wp-block-heading has-medium-font-size\">Finally we will see decreased T3 and T4 while increased TSH.<\/h1>\n\n\n\n<h1 class=\"wp-block-heading has-medium-font-size\"><strong>If it is secondary hypothyroidism then T3 and T4 will be low because decreased TSH is released by Pituitary.<\/strong><\/h1>\n\n\n\n<h1 class=\"wp-block-heading has-medium-font-size\">&nbsp;<\/h1>\n\n\n\n<p class=\"has-medium-font-size\">We will therefore see decreased T3 and T4 and also decreased TSH.<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-pale-ocean-gradient-background has-background\" style=\"font-size:24px\"><strong>CAUSES OF HYPOTHYROIDISM<\/strong><\/h2>\n\n\n\n<h1 class=\"wp-block-heading has-medium-font-size\">PRIMARY HYPOTHYROIDISM<\/h1>\n\n\n\n<ul class=\"wp-block-list\">\n<li>\u00b7&nbsp;&nbsp;&nbsp;&nbsp; Hashimoto thyroiditis<\/li>\n\n\n\n<li>\u00b7&nbsp;&nbsp;&nbsp;&nbsp; Iodine deficiency (endemic goiter)<\/li>\n\n\n\n<li>\u00b7&nbsp;&nbsp;&nbsp;&nbsp; Post surgery<\/li>\n\n\n\n<li>\u00b7&nbsp;&nbsp;&nbsp;&nbsp; Drugs blocking synthesis or release of T4 (for example \u2013 Lithium, sulfonamides)<\/li>\n\n\n\n<li>\u00b7&nbsp;&nbsp;&nbsp;&nbsp; Thyroiditis<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading has-medium-font-size\"><strong>SECONDARY HYPOTHYROIDISM<\/strong><\/h2>\n\n\n\n<p>Any mass lesion in the brain which compresses the stalk eg. Lymphocytic hypophysitis<\/p>\n\n\n\n<h2 class=\"wp-block-heading alignwide has-blush-light-purple-gradient-background has-background\" id=\"we-re-a-studio-in-berlin-with-an-international-practice-in-architecture-urban-planning-and-interior-design-we-believe-in-sharing-knowledge-and-promoting-dialogue-to-increase-the-creative-potential-of-collaboration\" style=\"font-size:22px;line-height:1.1\">               RISK FACTORS OF HYPOTHYROIDISM<\/h2>\n\n\n\n<ol class=\"wp-block-list\">\n<li>\u00b7&nbsp;&nbsp;&nbsp;&nbsp; Autoimmune disorder may be linked with similar family history.&nbsp;<\/li>\n\n\n\n<li>\u00b7&nbsp;&nbsp;&nbsp;&nbsp; Living in iodine deficient areas which causes enlargement of thyroid gland in order to meet the demand called goiter. Example \u2013 in hilly areas.<\/li>\n\n\n\n<li>\u00b7&nbsp;&nbsp;&nbsp;&nbsp; Goitrogenic food as the main diet like Broccoli, cabbage, cauliflower.&nbsp;Lacking balanced diet.<\/li>\n\n\n\n<li>\u00b7&nbsp;&nbsp;&nbsp;&nbsp; Thyroiditis \u2013 It is inflammation of thyroid gland.<\/li>\n<\/ol>\n\n\n\n<h2 class=\"wp-block-heading has-medium-font-size\"><strong>Subacute thyoiditis \/ DeQuervain\u2019s&nbsp;&nbsp; thyroiditis<\/strong><\/h2>\n\n\n\n<p class=\"has-medium-font-size\">It is generally seen in females of age group 30 \u2013 50 years.
