It might look easy but it breaks people from within.<\/p>\n\n\n\n<p>DALY(Disability adjusted life years) that tells about burden of disease in community is maximum for <strong>Depression<\/strong>.<\/p>\n\n\n\n<p class=\"has-pale-ocean-gradient-background has-background\"><strong>VARIOUS ASPECTS OF DEPRESSION<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Single episode of depression in life.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>RDD (Recurrent Depressive Disorder)- patients have 2 or more episodes of depression.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Persistent depressive disorder \u2013 very mild sadness for greater than 2 years called Dysthymia.<\/li>\n<\/ul>\n\n\n\n<p>Chronic major depression- Depression persisting for greater than 2 years.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Seasonal Affective Disorder (SAD) \u2013 Person\u2019s depression is associated with a particular season, usually it is winter.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Pre-menstrual dysphoric disorder<\/li>\n<\/ul>\n\n\n\n<p>In some, before menstruation there occurs low mood state which affects the quality of life and hinders proper functioning.<\/p>\n\n\n\n<p class=\"has-blush-light-purple-gradient-background has-background\"><strong>SYMPTOMS OF DEPRESSION<\/strong><\/p>\n\n\n\n<p>According to DSM, 5 out of 9 following features are required to diagnose depression.<\/p>\n\n\n\n<p>Out of which either depressed mood or anhedonia (loss of interest) must be present.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Anhedonia<\/strong>&#8211; loss of interest in previously pleasurable activities.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Depressed mood (more than 2 weeks of sadness)<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Unnecessary\/ too much guilt \u2013 pathological guilt.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Lack of energy \u2013 feeling tiredness all time.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Early morning awakening is seen.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Decreased attention in work or anything.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>There is decreased appetite and furthermore weight loss.<\/li>\n<\/ul>\n\n\n\n<p><strong>Significant weight loss<\/strong> is characteristic of depression that is at least 5% decrease in ideal body weight in 6 months.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Psychomotor activity like retardation\/ agitation.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Suicidal thoughts.<\/li>\n<\/ul>\n\n\n\n<p class=\"has-pale-ocean-gradient-background has-background\"><strong>GRADING OF DEPRESSION ACCORDING TO ICD11 \u2013<\/strong><\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"1024\" height=\"1015\" src=\"https:\/\/halleysclinic.com\/wp-content\/uploads\/2023\/05\/20230520_124609-1024x1015.jpg\" alt=\"Grading of depression\" class=\"wp-image-80\" srcset=\"https:\/\/halleysclinic.com\/wp-content\/uploads\/2023\/05\/20230520_124609-1024x1015.jpg 1024w, https:\/\/halleysclinic.com\/wp-content\/uploads\/2023\/05\/20230520_124609-300x297.jpg 300w, https:\/\/halleysclinic.com\/wp-content\/uploads\/2023\/05\/20230520_124609-150x150.jpg 150w, https:\/\/halleysclinic.com\/wp-content\/uploads\/2023\/05\/20230520_124609-768x761.jpg 768w, https:\/\/halleysclinic.com\/wp-content\/uploads\/2023\/05\/20230520_124609-600x595.jpg 600w, https:\/\/halleysclinic.com\/wp-content\/uploads\/2023\/05\/20230520_124609-100x100.jpg 100w, https:\/\/halleysclinic.com\/wp-content\/uploads\/2023\/05\/20230520_124609.jpg 1340w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><\/figure>\n\n\n\n<p class=\"has-blush-light-purple-gradient-background has-background\"><strong>ATYPICAL DEPRESSION<\/strong><\/p>\n\n\n\n<p>This is other type of depression-<\/p>\n\n\n\n<p>It is associated with hypersomnia, obesity, and increased carbohydrate craving with increased eating pattern.<\/p>\n\n\n\n<p><strong>Leaden paralysis is seen that is <\/strong>sense of feeling heavy and because of which they don\u2019t move around.<\/p>\n\n\n\n<p>They become upset over small things. There is increased interpersonal sensitivity.<\/p>\n\n\n\n<p class=\"has-pale-ocean-gradient-background has-background\"><strong>RISK FACTORS FOR DEPRESSION<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Family history and genetic predisposition.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Neurotransmitters like serotonin, norepinephrine and dopamine are <strong>decreased<\/strong> in depression.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Social factors like \u2013<\/strong><\/li>\n<\/ul>\n\n\n\n<p>Relationship issues<\/p>\n\n\n\n<p>Financial issues<\/p>\n\n\n\n<p>No job<\/p>\n\n\n\n<p>Living alone and away from family.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Loss of loved ones, not being able to forget the incident and unable to move on.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Persons expect a lot from themselves.<\/li>\n<\/ul>\n\n\n\n<p>When they fail to achieve so, they get angry from themselves and are not able to do anything about it.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Regret and guilt and thinking that you were the reason\/ cause of what happened.<\/li>\n<\/ul>\n\n\n\n<p>If you did this or that.<\/p>\n\n\n\n<p>If you said this or that.<\/p>\n\n\n\n<p>Getting angry and depressed over what already had happened.<\/p>\n\n\n\n<p>&nbsp;(Introjection \u2013 blaming oneself)<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Ambivalence is inability to decide between two. When there are two ways and we are not sure which one to take.<\/li>\n<\/ul>\n\n\n\n<p>We are stuck in a loop of thoughts, which becomes like a marsh wherein the more we think, the more we are trapped.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Aaron Beck<\/strong> postulated cognitive triad of depression which he described as <strong>thought errors<\/strong>.<\/li>\n<\/ul>\n\n\n\n<p><strong>These are-<\/strong><\/p>\n\n\n\n<p>Hopelessness \u2013 Person is not able to see a future.<\/p>\n\n\n\n<p>Helplessness<\/p>\n\n\n\n<p>Worthlessness \u2013 person feels that she can\u2019t do anything but on other hand she is fully capable.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Hypothyroidism and deficiency of certain vitamins like vitamin B12, vitamin D and folic acid are also associated with depression.<\/li>\n<\/ul>\n\n\n\n<p class=\"has-blush-light-purple-gradient-background has-background\"><strong>Management of depression<\/strong><\/p>\n\n\n\n<p>Antidepressants like SSRIs and SNRIs are given.<\/p>\n\n\n\n<p>SNRIs are given when motivation and energy level is low.<\/p>\n\n\n\n<p>Psychological approaches involve-<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>CBT<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Interpersonal psychotherapy-<\/li>\n<\/ul>\n\n\n\n<p>It is directed to improve communication and help seeking attitude in the individual.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Solution focused therapy-<\/li>\n<\/ul>\n\n\n\n<p>Solution of the problem or cause of depression is looked for and then solving it.<\/p>\n\n\n\n<p>Depression should never be ignored, be it you or someone in your family.<\/p>\n\n\n\n<p>Consult a specialist as and when it is required.<\/p>\n\n\n\n<p>In front of people, you can hide behind a pretty smile but you can never do so from yourself. Only you know that things are not right.<\/p>\n\n\n\n<p>You must always remember that no matter at which block you are stuck, with the time you will move ahead. You won\u2019t just walk but will also run.
