AK RANA<\/strong><strong><\/strong><\/li>\n<\/ul>\n\n\n\n<p>Diabetes is a Greek work meaning siphon &#8211; to pass through, and and has been known since old times.<\/p>\n\n\n\n<p>Non-communicable diseases like Diabetes have become so much common in population that it no longer looks like a big thing.<\/p>\n\n\n\n<p>Although, with advancement in medical science, there is nothing to be afraid of; still they do affect the quality of our life to great extent. It\u2019s required to be screened on regular basis to avoid it or to treat it at an early stage, so that the major problems like organ damage that follows it and become the cause of morbidity, do not come our way.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><strong>INSULIN, GLUCOSE, &amp; DIABETES<\/strong><\/p>\n\n\n\n<p class=\"has-pale-ocean-gradient-background has-background\">Increase in blood glucose leads to insulin release and that leads to decrease in blood glucose levels (insulin enable cells taking up the glucose from the blood). Random blood sugar (RBS) more than 200 mg\/dl with symptoms<\/p>\n\n\n\n<p class=\"has-pale-cyan-blue-background-color has-background\">    4.  HbA1c more than 6.5%<\/p>\n\n\n\n<p>\u00a0\u00a0\u00a0<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><\/p>\n\n\n\n<p>\u00a0\u00a0 <strong>RISK FACTORS<\/strong><\/p>\n\n\n\n<ol class=\"wp-block-list\" type=\"1\">\n<li>Centripetal obesity &#8211;&nbsp; waist circumference &gt;102 cm in males, &gt;88 cm in females<\/li>\n<\/ol>\n\n\n\n<p class=\"has-cyan-bluish-gray-background-color has-background\">&gt;90 cm in males, &gt;80cm in females (in Asians)<\/p>\n\n\n\n<p>It causes insulin resistance; cells do not take up glucose leading to increase in blood glucose.<\/p>\n\n\n\n<p class=\"has-background\" style=\"background:linear-gradient(135deg,rgba(255,155,72,0) 0%,rgb(254,45,45) 50%,rgb(107,0,62) 100%)\">[Obesity leads to lipolysis, formation of triglycerides which are converted to FFA (free fatty acids) that are substrate for new glucose formation and insulin resistance)]<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>High- fat diet<\/li>\n\n\n\n<li>Sedentary lifestyle, lack of physical activity.<\/li>\n\n\n\n<li>Family history of diabetes or autoimmune diseases.<\/li>\n<\/ul>\n\n\n\n<p><\/p>\n\n\n\n<p><\/p>\n\n\n\n<p class=\"has-pale-ocean-gradient-background has-background\">Why increase in blood glucose such a major concern?<\/p>\n\n\n\n<p class=\"has-blush-light-purple-gradient-background has-background\">It is because this glucose leads to defective metabolism of carbohydrate, protein; damage liver, kidney and our major blood vessels, that further leads to complications<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><\/p>\n\n\n\n<p class=\"has-white-background-color has-background\">2.   Symptoms like polyuria, polydipsia, polyphagia.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Increased tiredness<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Increase in chance of infection ( due to defective protein metabolism like<\/li>\n<\/ul>\n\n\n\n<p class=\"has-electric-grass-gradient-background has-background\">Rhinocerebral mucormycosis<\/p>\n\n\n\n<p class=\"has-blush-light-purple-gradient-background has-background\">Malignant otitis externa \u2013 presents with earache at night.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Edema<\/li>\n<\/ul>\n\n\n\n<p>Due to glycosuria( excretion of glucose in urine) there is increased uptake of salt and water in kidney)<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p class=\"has-very-light-gray-to-cyan-bluish-gray-gradient-background has-background\">3. Diabetic ketoacidosis \u2013 patient presents with vomiting, tachycardia, abdominal pain, difficulty in breathing. ( emergency condition)<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p>4. Urinary tract infection<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p>5. Hypertension<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p>6. Atherosclerosis<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p>7.  Retinopathy- can lead to vision loss<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><strong><u>MANAGEMENT OF DIABETES MELLITUS<\/u><\/strong><\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><strong>PHARMACOLOGICAL MANAGEMENT<\/strong><\/p>\n\n\n\n<p><\/p>\n\n\n\n<p>Insulin \u2013<\/p>\n\n\n\n<p>&nbsp;Rapid acting: Aspart, Lispro, Glulisine<\/p>\n\n\n\n<p>&nbsp; &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Intermediate acting: NPH<\/p>\n\n\n\n<p>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Long acting: Detemir, Glargine<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Biguanides- Metformin<\/li>\n\n\n\n<li>SGLT-2 inhibitors \u2013 Dapagliflozin<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>GLP-1 receptor agonists- Liraglutide<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>DPP-4 inhibitors<\/li>\n<\/ul>\n\n\n\n<p><\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><strong>NON- PHARMACOLOGICAL MANAGEMENT<\/strong><\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Carbohydrate content should be 50-60% of total calorie intake.<\/li>\n<\/ul>\n\n\n\n<p><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Fiber intake should be around 25-40 g per day.<\/li>\n<\/ul>\n\n\n\n<p><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Protein intake should be 15% of total calorie intake.<\/li>\n<\/ul>\n\n\n\n<p><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Fat intake should be limited to less than 30%<\/li>\n<\/ul>\n\n\n\n<p><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Non vegetarians should consume 100-200g of fish\/week (rich source of PUFA)<\/li>\n<\/ul>\n\n\n\n<p><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Vegetarians should consume vegetable oils ( soyabean\/ safflower\/ sunflower), walnuts and flaxseeds.<\/li>\n<\/ul>\n\n\n\n<p><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Salt consumption should be less than 5g per day.<\/li>\n<\/ul>\n\n\n\n<p><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Patients should be advised to avoid smoking, alcohol intake, sugars and sweetened beverages.<\/li>\n<\/ul>\n\n\n\n<p><\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><strong>PREVENTION OF DIABETES<\/strong><\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>A minimum of <strong>150 min\/week <\/strong>of physical activity is required. This should involve \u2013<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>\u2265 30 min of moderate- intensity aerobic activity each day.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>15-30 min of work-related activity.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>15 min of muscle-strengthening exercises ( at least 3 times\/week )<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>To maintain a good sleep-wake cycle. It keeps our body in rhythm, maintains basal metabolic rate and prevents hormonal changes.<\/li>\n<\/ul>\n\n\n\n<p class=\"has-blush-light-purple-gradient-background has-background\">As you see, in our body one key is linked to another and the chain continues.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>In such a busy life, full of stress, time should be or I will say must be taken out to relieve stress by following your hobby, talking to friends and family, taking a walk under wide sky, listening to good music or joining yoga classes.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Not only in adults, but obesity and lack of physical activity in children also, can create a risk factor for diabetes. They should be given more of outdoor activities daily rather than playing games at home.<\/li>\n<\/ul>\n\n\n\n<p class=\"has-pale-ocean-gradient-background has-background\">Besides generating a healthy attitude at young age will stay with stay with them always and doing daily yoga and exercises will become habit over the time.<\/p>\n\n\n\n<p>Food as I stated before plays a vital role.
