AK RANA<\/li>\n<\/ul>\n\n\n\n<p>Incidence of HCC (Hepatocellular carcinoma) is increasing. It occurs more in males than females. Improper lifestyle, lack of balanced diet and regular exercises can be seen as a factor in general, along with various other important causes.<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-pale-ocean-gradient-background has-background\"><strong><u>RISK FACTORS OF HCC<\/u><\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Hepatitis B virus (chronic hepatitis)<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Hepatitis C virus<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong><u>Alcohol <\/u><\/strong>\u2013 it can cause Alcoholic liver disease (ALD), which can cause cirrhosis. Intake of 60 ml of alcohol per day for 10 years can lead to ALD.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Obesity<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong><u>Aflatoxin exposure \u2013 <\/u><\/strong>Aflatoxin is produced by fungi Aspergillus and found on certain crops like maize, peanuts etc.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>NASH (Non- alcoholic steatohepatitis)<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>NAFLD (Non- alcoholic fatty liver disease)<\/li>\n<\/ul>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"699\" height=\"1024\" src=\"https:\/\/halleysclinic.com\/wp-content\/uploads\/2023\/08\/image-6-699x1024.png\" alt=\"hepatocellular carcinoma  disease progression\n\n\nhalleys clinic\" class=\"wp-image-207\" srcset=\"https:\/\/halleysclinic.com\/wp-content\/uploads\/2023\/08\/image-6-699x1024.png 699w, https:\/\/halleysclinic.com\/wp-content\/uploads\/2023\/08\/image-6-205x300.png 205w, https:\/\/halleysclinic.com\/wp-content\/uploads\/2023\/08\/image-6-768x1126.png 768w, https:\/\/halleysclinic.com\/wp-content\/uploads\/2023\/08\/image-6-600x880.png 600w, https:\/\/halleysclinic.com\/wp-content\/uploads\/2023\/08\/image-6.png 940w\" sizes=\"auto, (max-width: 699px) 100vw, 699px\" \/><\/figure>\n\n\n\n<h2 class=\"wp-block-heading has-pale-ocean-gradient-background has-background\"><strong>STAGES OF DISEASE PROGRESSION<\/strong><\/h2>\n\n\n\n<p>In Alcoholic liver disease there is history of alcohol intake.<\/p>\n\n\n\n<p>In NASH\/NAFLD, there is history of obesity, Diabetes mellitus, hypercholesterolemia.<\/p>\n\n\n\n<p>In both of the above the cases (in general) \u2013<\/p>\n\n\n\n<p><strong>STAGE 1<\/strong> \u2013 Fatty change is seen in hepatocytes\/ liver cells.<\/p>\n\n\n\n<p>(This stage is reversible)<\/p>\n\n\n\n<p><strong>STAGE 2<\/strong> \u2013 Hepatitis stage<\/p>\n\n\n\n<p>Inflammation of liver parenchyma.<\/p>\n\n\n\n<p>(Infiltration of neutrophils\/ monocytes)<\/p>\n\n\n\n<p><strong>Cirrhosis stage<\/strong> \u2013 starting from mild fibrosis to bridging fibrosis that connects different portal tracts.<\/p>\n\n\n\n<p>From cirrhosis, various directions can be taken by the disease \u2013<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Liver failure<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Portal hypertension (HTN)<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>HCC (Hepatocellular carcinoma)<\/strong><\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading has-pale-ocean-gradient-background has-background\"><strong>CLINICAL FEATURES OF HCC<\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Hepatomegaly<\/li>\n<\/ul>\n\n\n\n<p>Liver is increased in size; patient complains about presence of an abdominal lump.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Jaundice due to abnormal liver function.<\/li>\n<\/ul>\n\n\n\n<p>It usually presents late.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Weight loss and weakness is seen.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Hypoglycaemia (Low blood sugar level) is seen.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Hypercholesterolemia \u2013 Increased level of cholesterol is seen.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading has-pale-ocean-gradient-background has-background\"><strong><u>DIAGNOSIS<\/u><\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong><u>LFT (Liver function test)<\/u><\/strong><\/li>\n\n\n\n<li>ALT (Alanine aminotransferase) and AST (Aspartate aminotransferase) generally their values &lt;30 units\/l is considered normal.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Normal bilirubin up to 1.2 mg\/dl<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Alkaline phosphatase<\/li>\n<\/ul>\n\n\n\n<p>Normal = 40-140 IU\/l<\/p>\n\n\n\n<p>Abnormal = &gt;4 times normal<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong><u>USG (Ultrasonography)<\/u><\/strong><\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong><u>Triple phase CT scan<\/u><\/strong> is IOC (Investigation of choice)<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Non- contrast phase<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Arterial phase<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Washout\/ venous phase<\/li>\n<\/ul>\n\n\n\n<p>It is done to differentiate between HCC &amp; metastasis (e.g., From colorectal carcinoma)<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong><u>Tru cut\/ core needle biopsy<\/u><\/strong><\/li>\n<\/ul>\n\n\n\n<p>It can be done to confirm the diagnosis.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Various <strong><u>tumour markers<\/u><\/strong> are \u2013<\/li>\n<\/ul>\n\n\n\n<ol class=\"wp-block-list\" type=\"1\">\n<li>Alfa feto protein<\/li>\n\n\n\n<li>Glycipan<\/li>\n\n\n\n<li>HepPar-1<\/li>\n\n\n\n<li>Neurotensin B<\/li>\n<\/ol>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong><u>Fibro scan<\/u><\/strong><\/li>\n<\/ul>\n\n\n\n<p>If it shows increased stiffness then cirrhosis is likely to be present.<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-pale-ocean-gradient-background has-background\"><strong><u>WHAT WE CAN DO?<\/u><\/strong><\/h2>\n\n\n\n<p>After knowing HCC, the most important question is how to prevent its occurrence or how to protect ourselves and our family from it.<\/p>\n\n\n\n<p>The answer lies in the risk factors and the mechanism that follows it.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong><u>Stop drinking alcohol<\/u><\/strong><\/li>\n<\/ul>\n\n\n\n<p>At least when your LFT (liver function test) is abnormal or when your doctor writes in your report \u2018FATTY LIVER\u2019.<\/p>\n\n\n\n<p>Fatty liver in the report is a \u2018warning\u2019 that liver is weak and you need to heal it.<\/p>\n\n\n\n<p>It is a reversible situation and with less fats, more exercise, no alcohol you can change it with the time.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong><u>Proper immunization of Hepatitis<\/u><\/strong><\/li>\n<\/ul>\n\n\n\n<p>As through National Immunization schedule, it is being strictly followed; hence, cases arising through chronic hepatitis is decreasing.<\/p>\n\n\n\n<p>Still, if you are not immune, you should consult your doctor for proper guidance.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong><u>Obesity and high cholesterol level<\/u><\/strong><\/li>\n<\/ul>\n\n\n\n<p>Our food habits are to be blamed for this. Eating instant, high sugar, oily or food with \u2018extra cheese\u2019 or saturated fats are not good for your liver or your heart.<\/p>\n\n\n\n<p>Eat a balanced diet.<\/p>\n\n\n\n<p>Take timely and proper meals.
